JUC-357 - Wife Being Ravished While Her Husband Watches, Maki Mizusawa

The premise is that the husband is bedridden due to a traffic accident, leading the actress to seek an outlet for her desires. In the scenes with her brother-in-law, she resists at first but eventually ends up on top, grinding away in a cowgirl position—which I think is the highlight of the film. However, it’s nowhere near the 'rape' theme the title suggests. On top of that, the fact that they were former lovers makes the whole thing even more confusing. In the end, she even gets involved with the person who caused the accident, and it wraps up with a 3P and them successfully collecting the insurance money—a 'happily ever after' of sorts. Since the actress seems to have satisfied her urges, the whole thing was completely disconnected from the 'reluctant/forced' vibe the title implies.
