Okay, so this is just my personal take, but... I bought this imagining all sorts of things based on Sakura-san's 'handjob' theme, but it kinda shifts from that into a romance as it goes on... The title 'Handjob Substitute' made me expect something along those lines, something in the vein of her previous 'a little perverted' works. So when it deviates from that theme, I just kinda lose interest... Maybe the title is the problem. 'Handjob Substitute' makes you imagine and expect something specific... 'Love Starting with a Handjob' might have been a better title. The actresses don't tend to revisit the same themes in multiple works. This might end up being the definitive 'handjob' themed work in Sakura-san's filmography... I like the theme itself, being the pervert that I am, but it's a little disappointing.
